Additional information: Het maximale bestelbedrag werd hoger gezet dan de raming aangezien het realistisch is dat Zorgband Leie en Schelde nog zal uitbreiden in de toekomst. Op die manier kunnen ook nieuwe deelgenoten afnemen van deze raamovereenkomst zonder dat het maximale bestelbedrag onmiddellijk overschreden wordt.

Description of selection criterion: Bankattest: dit is een bevestiging van de financiële instelling dat de inschrijver houder is van de bankrekening waarop de facturen voor deze opdracht moeten betaald worden. Minimumeisen: De inschrijver moet een recent bankattest (max. 6 maand oud) toevoegen aan de offerte. Indien er geen (recent) attest aan de offerte werd toegevoegd, wordt de offerte als substantieel onregelmatig beschouwd en bijgevolg nietig verklaard.
Criterion: References on specified services
Description of selection criterion: Een lijst van minimum 3 en maximum 5 referenties van gelijkaardige opdrachten die gedurende de laatste drie jaar werden verricht, met vermelding van het bedrag, de datum en de publiek- of privaatrechtelijke instanties waarvoor zij bestemd waren. Minimumeisen: De inschrijver moet een overzicht van deze referenties aan de offerte toevoegen met vermelding van de contactgegevens van minstens 1 referentiepersoon (naam, e-mail, tel). Wanneer er geen referenties werden opgegeven of de opgegeven referenties niet correct blijken te zijn of niet voldoen aan de verwachtingen, is het mogelijk om de offerte substantieel onregelmatig en bijgevolg nietig te verklaren.

Description of selection criterion: Procedure werken met derden. Deze procedure beschrijft de richtlijnen en stappen voor het samenwerken met derden om de kwaliteit, de veiligheid en gezondheid én de naleving van de regelgeving te waarborgen. Dit document bevat de voorschriften en richtlijnen rond veiligheid, gezondheid en milieu (VGM) op alle sites van Zorgband Leie & Schelde. Minimumeisen: De inschrijver gaat akkoord met deze procedure en bezorgd een ondertekende versie hiervan terug. Indien geen ondertekend exemplaar aan offerte werd toegevoegd, wordt de offerte als substantieel onregelmatig beschouwd en bijgevolg nietig verklaard.

Criterion: Professional risk indemnity insurance
Description of selection criterion: Het bewijs van een verzekering tegen beroepsrisico's. Minimumeisen: De inschrijver moet het bewijs van een verzekering tegen beroepsaansprakelijkheidsrisico's en burgerlijke aansprakelijkheid toevoegen aan de offerte. Het bewijs van verzekering moet nog geldig zijn op datum van opening van de offertes. Indien geen geldig bewijs aan de offerte werd toegevoegd, wordt de offerte als substantieel onregelmatig beschouwd en bijgevolg nietig verklaard.
