Description of selection criterion: Folgende Eignungsnachweise sind dem Angebot beizufügen: 1. Eigenerklärung über die ordnungsgemäße Erfüllung der Verpflichtung zur Zahlung von Steuern und Abgaben; 2. Eigenerklärung über die ordnungsgemäße Anmeldung und Erfüllung der Verpflichtung zur Entrichtung der gesetzlichen Beiträge bei der/den Krankenkasse(n); 3. Eigenerklärung über die ordnungsgemäße Eintragung und Erfüllung der Verpflichtung zur Entrichtung der gesetzlichen Beiträge bei der Berufsgenossenschaft. 4. Eigenerklärung zum Nichtvorliegen von Ausschlussgründen nach GWB. 5. Datenschutzverpflichtungserklärung Entsprechende Formblätter liegen den Vergabeunterlagen bei. Stichtag für die Gültigkeit ist der Schlusstermin für den Eingang der Angebote. Eigenerklärungen dürfen an diesem Stichtag nicht älter als 3 Monate, jeweils gerechnet ab Ausstellungsdatum sein. Werden anstelle von Eigenerklärungen Unbedenklichkeitsbescheinigungen der zuständigen Stellen eingereicht und ist in diesen keine Gültigkeitsdauer angegeben, dürfen die Bescheinigungen an diesem Stichtag nicht älter als ein Jahr sein.
