Description: 1. Przedmiotem zamówienia są Dostawy sukcesywne produktów leczniczych w ramach Programów Leczniczych , Chemioterapii, RDTL ,Leków Ogólnych i Importu na potrzeby Klinik/Oddziałów Centralnego Szpitala Klinicznego UM w Łodzi – przy ul. Pomorskiej 251 2. Zamówienie zostało podzielone na 79 części. 3. Szczegółowy opis przedmiotu zamówienia stanowi Załącznik nr 1 do SWZ – Formularz asortymentowo - cenowy. 4. Termin realizacji zamówień standardowych – w ciągu max. 48 godzin (dni robocze pn-pt) 5. Termin realizacji zamówień pilnych – w ciągu max. 24 godzin (dni robocze pn-pt). 6. Termin realizacji zamówień „Na Ratunek Życia” – w ciągu max 6 godzin (Zamawiający wymaga dostawy produktów leczniczych w trybie Na Ratunek Życia w soboty, niedziele i w święta oraz bezpośredniego numeru kontaktowego z osobą realizującą zamówienie od strony Wykonawcy) – w przypadku wątpliwości czy dany lek jest przez Zamawiającego traktowany jako lek „na ratunek życia” (o ile dotyczy). 7. W/w warunki nie dotyczą leków sprowadzanych w trybie importu docelowego. 8. Wykonawca zobowiązany jest zrealizować zamówienie na zasadach i warunkach opisanych we wzorze umowy stanowiącym Załącznik nr 2 do SWZ w zależności na którą część składa ofertę. 9. Termin ważności dostarczanych produktów: a. dla produktów leczniczych min. 12 m-cy licząc od dnia dostawy, chyba, że Zamawiający wskazał inaczej w informacjach pod tabelą w Formularzu cenowym – załącznik nr 2 do SWZ; b. dla kontrastów min. 12 m-cy licząc od dnia dostawy (o ile dotyczy); c. dla wyrobów medycznych min. 12 m-cy licząc od dnia dostawy (o ile dotyczy); d. dla produktów do żywienia dojelitowego i pozajelitowego, żywności przeznaczonej dla niemowląt i małych dzieci oraz produktów spożywczych specjalnego przeznaczenia medycznego termin ważności winien być w dniu dostawy nie krótszy niż połowa terminu ważności gwarantowanego przez producenta (o ile dotyczy). 10. Wspólny Słownik Zamówień CPV: 33690000-3 Różne produkty lecznicze 33600000-6 Produkty farmaceutyczne 33692000-7 Roztwory lecznicze 11. Minimalna wartość lub wielkość świadczenia jaką zamawiający zobowiązuje się zrealizować w ramach umowy, zgodnie z formularzem asortymentowo cenowym. Należy uwzględnić opcję zwiększenia zamówienia o dodatkowe ilości ,zgodnie z formularzem asortymentowo cenowym. 12. Zamawiający dopuszcza możliwość składania ofert częściowych. Wykonawca może złożyć ofertę na jedną lub więcej części. 13. Ewentualne użycie w dokumentacji określeń i nazw własnych ma jedynie charakter przykładowy i służy określeniu klasy i jakości materiałów. Jeżeli w opisie przedmiotu zamówienia znajdują się wskazania znaków towarowych, patentów lub pochodzenia, źródła lub szczególnego procesu, który charakteryzuje produkty lub usługi dostarczane przez konkretnego Wykonawcę, jeżeli mogłoby to doprowadzić do uprzywilejowania lub wyeliminowania niektórych Wykonawców lub produktów, chyba że jest to uzasadnione specyfiką przedmiotu zamówienia i Zamawiający nie może opisać przedmiotu zamówienia za pomocą dostatecznie dokładnych określeń, a wskazaniu takiemu towarzyszą wyrazy „lub równoważny”., Wykonawca może zaoferować przedmioty równoważne. Zamawiający wymaga, aby użyte materiały, o ile są inne, posiadały parametry jakościowe i techniczne nie gorsze niż określone w przedmiocie zamówienia. Wykazanie równoważności zaoferowanego przedmiotu spoczywa na Wykonawcy. 14. Zamawiający nie dopuszcza możliwości składania ofert wariantowych. 15. Zamawiający nie dopuszcza składania ofert w postaci katalogów elektronicznych. 16. Zamawiający nie przewiduje zawarcia umowy ramowej. 17. Zamawiający nie zastrzega obowiązku osobistego wykonania przez Wykonawcę kluczowych części zamówienia. 18. Wykonawca może powierzyć wykonanie części zamówienia podwykonawcy. Zamawiający żąda wskazania przez Wykonawcę części zamówienia, których wykonanie zamierza powierzyć podwykonawcom, i podania przez Wykonawcę firm tych podwykonawców o ile są znane na tym etapie postępowania. 19. Jeżeli zmiana albo rezygnacja z podwykonawcy dotyczy podmiotu, na którego zasoby Wykonawca powoływał się, na zasadach określonych w art. 118 ust. 1 ustawy PZP, w celu wykazania spełniania warunków udziału w postępowaniu, Wykonawca jest obowiązany wykazać Zamawiającemu, że proponowany inny podwykonawca lub Wykonawca samodzielnie spełnia je w stopniu nie mniejszym niż podwykonawca, na którego zasoby Wykonawca powoływał się w trakcie postępowania o udzielenie zamówienia. 20. Zamawiający nie przeprowadził wstępnych konsultacji rynkowych. 21. Zamawiający nie przewiduje odbycia wizji lokalnej przez Wykonawcę i sprawdzenia przez niego dokumentów niezbędnych do realizacji zamówienia.

Additional information: INFORMACJA O PRZEDMIOTOWYCH ŚRODKACH DOWODOWYCH 1.W celu potwierdzenia, że oferowane dostawy są zgodne z wymaganymi cechami zgodnie z art. 104-106 Ustawy, Zamawiający wymaga złożenia wraz z ofertą następujących przedmiotowych środków dowodowych: a) Oświadczenie Wykonawcy (Załącznik nr 7 do SWZ) potwierdzające, że zaoferowane wyroby spełniają wymagane warunki określone w SWZ, i posiadają wszystkie dokumenty wymienione w opisie przedmiotu zamówienia w Załączniku nr 1 do SWZ oraz posiadają dokumenty potwierdzające dopuszczenie do obrotu na rynek polski oferowanych produktów /jeśli dotyczy/ b)Dokument potwierdzający opis przedmiotu zamówienia pochodzący od producenta/wytwórcy/autoryzowanego przedstawiciela lub informacja wygenerowana elektronicznie z oficjalnego portalu /strony producenta. Przepisu ust. 2 nie stosuje się, jeżeli przedmiotowy środek dowodowy służy potwierdzeniu zgodności z cechami lub kryteriami określonymi w opisie kryteriów oceny ofert lub, pomimo złożenia przedmiotowego środka dowodowego, oferta podlega odrzuceniu albo zachodzą przesłanki unieważnienia postępowania. 4. UE nr L 229 z 31.7.2014, str. 1) w brzmieniu nadanym rozporządzeniem 2022/576 Z postępowania o udzielenie zamówienia Zamawiający wykluczy Wykonawcę: 1) będącego obywatelem rosyjskim lub osobą fizyczną lub prawną, podmiotem lub organem z siedzibą w Rosji; 2) będącego osobą prawną, podmiotem lub organem, do których prawa własności bezpośrednio lub pośrednio w ponad 50 % należą do podmiotu, o którym mowa w pkt 1; c) będącego osobą fizyczną lub prawną, podmiotem lub organem działającym w imieniu lub pod kierunkiem podmiotu, o którym mowa w pkt 1 lub 2, w tym podwykonawców, dostawców lub podmiotów, na których zdolności polega się w rozumieniu dyrektyw w sprawie zamówień publicznych, w przypadku gdy przypada na nich ponad 10 % wartości zamówienia. Zamawiający ocenia, czy podjęte przez Wykonawcę czynności, o których mowa w pkt. 3), są wystarczające do wykazania jego rzetelności, uwzględniając wagę i szczególne okoliczności czynu Wykonawcy. Jeżeli podjęte przez Wykonawcę czynności, o których mowa w pkt. 3, nie są wystarczające do wykazania jego rzetelności, Zamawiający wyklucza Wykonawcę. Wykonawca może zostać wykluczony przez Zamawiającego na każdym etapie postępowania o udzielenie zamówienia Wykluczenie Wykonawcy następuje zgodnie z art. 111 ustawy Pzp. 4.Zamawiający ocenia, czy podjęte przez Wykonawcę czynności, o których mowa w art. 110 ust. 2, są wystarczające do wykazania jego rzetelności, uwzględniając wagę i szczególne okoliczności czynu Wykonawcy. Jeżeli podjęte przez Wykonawcę czynności, o których mowa w art. 110 ust. 2, nie są wystarczające do wykazania jego rzetelności, Zamawiający wyklucza Wykonawcę. 5. Wykluczenie Wykonawcy następuje zgodnie z art. 111 ustawy Pzp. 6. Wykonawca może zostać wykluczony przez Zamawiającego na każdym etapie postępowania o udzielenie zamówienia Jeżeli Zamawiający przewiduje wykluczenie Wykonawcy na podstawie ust. 1, wskazuje podstawy wykluczenia w ogłoszeniu o zamówieniu lub dokumentach zamówienia.
