Description of selection criterion: Een verklaring betreffende de totale omzet én de omzet van de bedrijfsactiviteit die het voorwerp van de opdracht is, over de laatste drie beschikbare boekjaren, afhankelijk van de oprichtingsdatum of van de datum waarop de ondernemer met zijn bedrijvigheid is begonnen, voor zover de betrokken omzetcijfers beschikbaar zijn. Minimumeisen: De minimale gemiddelde jaarlijkse omzet van de bedrijfsactiviteit die het voorwerp uitmaakt van deze opdracht, gerekend over de laatste drie beschikbare boekjaren, bedraagt €80.000.

Description of selection criterion: Een lijst van de voornaamste soortgelijke opdrachten die gedurende de afgelopen drie jaar werden uitgevoerd in België met vermelding van naam instelling, het bedrag, datum, contactpersoon en korte beschrijving van de opdracht. De diensten worden aangetoond door attesten die de bevoegde autoriteit heeft afgegeven of medeondertekend of, in geval van diensten voor een particuliere afnemer, door attesten van de afnemer of bij ontstentenis eenvoudigweg door een verklaring van de dienstverlener. Minimumeisen: Aanleveren van een referentielijst volgens de tabel die werd opgegeven in bijlage van dit bestek (vermelding van datum van de diensten, bedrag en instantie waarvoor de diensten bestemd waren). + minimum 3 referenties in ziekenhuizen of zorginstellingen waar de dienstverlener verantwoordelijk is voor volgende types van keuringen: Elektriciteit (hoog- en laagspanning), liften, branddetectiesystemen.

Description of selection criterion: De inschrijver dient geaccrediteerd te zijn en te voldoen aan alle vigerende wetgeving en normering en te beschikken over de nodige erkenningen m.b.t. de domeinen van toepassing: (niet limitatief) *) erkenning als EDTC conform de bepalingen van het KB 29 april 1999 en KB 9 januari 2011; *) accreditatie volgens de norm NBN EN ISO/IEC 17020 met BELAC certificatie type A; *) vigerende VLAREM regelgeving; *) erkenningen milieudeskundigen voor VLAREM; *) VCA-certificaat + werknemers dienen een externe VCA-opleiding gevolgd te hebben; Niet-Belgische inschrijvers dienen hun competentie en geschiktheid aan te tonen d.m.v. voorlegging van gelijkwaardige erkenningen. Minimumeisen: Alle vereiste én recente certificaten, accreditaties en/of (nominatieve) erkenningen dienen per inspectiedomein duidelijk aangetoond te worden door de inschrijver.
