Description: 1. Przedmiotem zamówienia jest zakup leków, środków o charakterze dietetycznym przeznaczenia medycznego, wyrobów medycznych do apteki szpitalnej SWPZZPOZ w Radomiu. 2. Zamawiający dopuszcza możliwości składania ofert częściowych. Zamówienie zostało podzielone na 18 części (zadań). 2.1. Części nie mogą być dzielone przez Wykonawców, oferty nie zawierające pełnego zakresu przedmiotu zamówienia określonego w zadaniu częściowym zostaną odrzucone. 2.2. Wykonawca może złożyć ofertę w odniesieniu do wszystkich części zamówienia. 2.3. Zamawiający nie określa maksymalnej liczby części, na które zamówienie może zostać udzielone temu samemu Wykonawcy 3. Termin związania ofertą 09.01.2027 r. 4. O udzielenie zamówienia mogą ubiegać się Wykonawcy, którzy nie podlegają wykluczeniu na podstawie art. 108 ust.1 oraz art. 109 ust. 1 pkt 4, Ustawy Pzp, art. 5k Rozporządzenia Rady UE 833/2014 oraz art. 7 ust. 1 ustawy z dnia 13 kwietnia 2022 r. o szczególnych rozwiązaniach w zakresie przeciwdziałania wspieraniu agresji na Ukrainę oraz służących ochronie bezpieczeństwa narodowego. 5. Dokumenty składane wraz z ofertą: 5.1. FORMULARZ OFERTY – wzór stanowi załącznik nr 3 do SWZ. 5.2. Formularz cenowy zgodnie załącznikiem nr 1 do SWZ . 5.3. OŚWIADCZENIE w formie jednolitego europejskiego dokumentu zamówienia (JEDZ), w przypadku wspólnego ubiegania się o zamówienie przez wielu wykonawców, oświadczenie, o którym mowa powyżej, składa każdy z wykonawców. 5.4. OŚWIADCZENIA WYKONAWCY/wykonawcy wspólnie ubiegającego się o udzielenie zamówienia dotyczące przesłanek wykluczenia z art. 5k Rozporządzenia 833/2014 wg wzoru określonego w Załączniku nr 4a do SWZ. 5.5. PEŁNOMOCNICTWO lub inny dokument potwierdzający umocowanie do reprezentowania wykonawcy – w przypadku gdy umocowanie osoby nie wynika z dokumentów rejestrowych (KRS, CEiDG lub innego właściwego rejestru). Warunek ten dotyczy również odpowiednio osoby działającej w imieniu wykonawców wspólnie ubiegających się o udzielenie zamówienia publicznego oraz podwykonawców. Pełnomocnictwo to musi w swej treści jednoznacznie wskazywać uprawnienie do reprezentowania w postępowaniu o udzielenie zamówienia albo do reprezentowania w postępowaniu i zawarcia umowy w sprawie zamówienia publicznego. Umocowanie wymagane jest na każdym etapie prowadzonego postępowania. 5.6. ODPIS LUB INFORMACJĘ z Krajowego Rejestru Sądowego, Centralnej Ewidencji i Informacji o Działalności Gospodarczej lub innego właściwego rejestru (jeżeli dotyczy) - w celu potwierdzenia, że osoba działająca w imieniu wykonawcy jest umocowana do jego reprezentowania.
