Title: Lasten yksilölliset ortoosit
Description: Hankinnan kohteena ovat lasten ja nuorten ortopediset jalkineet ja tukipohjalliset, jalkineiden muutostyöt, yksilölliset ortoosit ja tankokengät. Hankinta sisältää sekä tavara- että palveluhankintaa. Palvelun lopputuloksena asiakas (potilas/käyttäjä) saa käyttöönsä yksilöllisesti mitatun ja sovitetun ortopedisen jalkineen, tukipohjallisen, ortoosin tai tankokengät (valmiin tuotteen). Hankintaan sisältyy lisäksi jalkineiden muutostöitä, joihin sisältyy yksilöllistä palvelua. Lasten ja nuorten ortopediset jalkineet, tukipohjalliset, ortoosit, tankokengät ja jalkineiden muutostyöt myönnetään lääkinnällisen kuntoutuksen apuvälineenä ja ovat siten saajalleen/asiakkaalle maksuttomia. Maksusitoumus myönnetään ennen jalkineen, tukipohjallisen, ortoosin, tankokenkien tai jalkineen muutostyön hankintaa ja sen tulee olla potilaalla mukana tuotetta hankittaessa. Hankinta on jaettu kolmeen (3) maantieteelliseen alueeseen, jotka muodostavat seuraavat hankinnan osat: Osa 1: HUS-alue Osa 2: Kymenlaakson hyvinvointialue Osa 3: Etelä-Karjalan hyvinvointialue Tarjoaja voi osallistua kaikkiin tai valitsemiinsa osiin. Hankintaan sisältyvät seuraavat osa-alueet / tuoteryhmät: Tuoteryhmä 1. Yksilölliset ortopediset jalkineet Tuoteryhmä 2. Ortopediset jalkineet Tuoteryhmä 3. Jalkineiden muutostyöt Tuoteryhmä 4. Tukipohjalliset Tuoteryhmä 5. Yksilölliset nilkka- ja jalkateräortoosit Tuoteryhmä 6. Sääriortoosit Tuoteryhmä 7. Pitkät alaraajaortoosit Tuoteryhmä 8. Lantio- tai vartalo-osalliset ortoosit Tuoteryhmä 9. Tankokengät Tuoteryhmä 10. Vetoketjulliset yksilöllisesti muokattavat ortoosit Tuoteryhmä 11. Yksilöllisesti muokattavat puolivalmiit ortoosit Hankinnan kohde ja sitä koskevat vaatimukset on kuvattu tarkemmin tarjouspyynnössä ja sen liitteissä. Sopimus on voimassa määräaikaisena kolme (3) vuotta, minkä jälkeen sopimus on voimassa toistaiseksi. Kyseessä on useamman hankintayksikön yhteishankinta, joka toteutetaan hankintarenkaana. Tässä tarjouspyynnössä pyydettyjä tuotteita tulevat käyttämään: HUS-yhtymä, Etelä-Karjalan hyvinvointialue ja Kymenlaakson hyvinvointialue. Hankinnan ennakoitu kokonaisarvo (alv 0 %) on yhteensä noin 2 000 000 euroa (48 kuukautta). Näistä Kymenlaakson hyvinvointialueen ennakoitu kokonaisarvo on n. 20 % ja Etelä-Karjalan hyvinvointialueen ennakoitu kokonaisarvo on n. 15 %. Kyse on arviosta, eikä hankintayksikkö sitoudu ilmoitettuihin euro- tai kappalemääriin ja tarjouspyynnössä ja sen liitteissä ilmoitettuihin kulutusmääräarvioihin. Hankinnan kohteeseen sisältyvät myös tarjottuihin tuotteisiin liittyvät lisätuotteet, kuten lisätarvikkeet, jotka ilmoitetaan tarjouspyynnön liitteenä olevilla Tuoteliitteillä (Liitteet 2.1, 2.2 ja 2.3). Tilaaja voi tilata Tuoteliitteen ulkopuolisia tuotteita tilanteessa, jossa sopimuskauden aikana ilmenee, että tyhjentävänä luettelona kilpailutetusta Tuoteliitteestä on puuttunut yksittäinen Tilaajan tarvitsema tuote. Kyse on poikkeuksesta, joka voisi tulla kyseeseen esimerkiksi potilaan yksilöllisestä erityisestä tarpeesta. Tällainen Tuoteliitteen ulkopuolisia tuotteita koskeva lisätilaus tulee kyseeseen vain satunnaisissa tapauksissa. Sopimuskaudella satunnaisesti tilattavien Tuoteliitteen ulkopuolisten tuotteiden arvo on huomioitu hankinnan arvonlisäverottomassa kokonaisarvossa. Kuten edellä on esitetty, on Tilaajan tavoitteena, ettei tällaisia tilauksia ole tarpeen tehdä sopimuskauden aikana lainkaan. Mikäli Tilaaja tilaa Tuoteliitteen ulkopuolisia tuotteita, voi tällaisten tilausten yhteenlaskettu osuus olla enintään 10 prosenttia hankinnan ennakoidusta arvonlisäverottomasta kokonaisarvosta koko sopimuskauden ajan.
