Ilości, charakterystykę oraz parametry techniczno-użytkowe przedmiotu zamówienia określono w opisie przedmiotu zamówienia – Załącznik nr 2-1 do 2-5 oraz Załącznikach nr 1, 5, 9 do specyfikacji warunków zamówienia (SWZ). 4. Oferowany przedmiot zamówienia powinien spełniać wymagania określone przez Zamawiającego w (jeżeli dotyczy): a) Ustawie o Wyrobach Medycznych z dnia 7.04.2022r. (tekst jednolity Dz.U. 2022poz. 974 z póz. zm.), b) Rozporządzenie Ministra Zdrowia z dnia 17 lutego 2016 r. w sprawie wymagań zasadniczych oraz procedur oceny zgodności wyrobów medycznych (Dz.U. z 2016r. poz. 211 z póżn. zm.); Rozporządzenie Parlamentu Europejskiego i Rady (UE) 2017/745 z dnia 5 kwietnia 2017 r. w sprawie wyrobów medycznych (MDR), oraz aktualne właściwe przepisy przejściowe dopuszczających dany wyrób do obrotu i używania na terytorium RP, c) winien spełniać wymagania techniczne zawarte w SWZ i załącznikach do SWZ, W tym celu Wykonawcy złożą: - oświadczenie, że oferowany przedmiot zamówienia posiada ważne dokumenty dopuszczające do obrotu i używania wystawione zgodnie z przepisami prawa - Załącznik nr 8.
