Palliative in-home care for people with life-threating illness.

The service is provided by in-home visits and is a palliative care (end of life care) for adults who are approaching the end of their live, with advanced, progressive incurable conditions. Palliative care provided shall be based on the international definition by World Health Organization (WHO):

‘Palliative care is an approach that improves the quality of life of patients and their families facing the problem associated with life-threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychosocial and spiritual’.

The service also includes final stages of palliative care, care of dying adults in the last days of life.

The nursing team consist of palliative care specialist nurses, and the team needs to be in good contact and coordinated with the regional hospital health care team, as well as other health care and social service that the patients will receive.

Practitioners and operators are obliged to have appropriate licenses and insurance according to Icelandic laws and regulations.

Duration of the contract or the framework agreement is 36 months (with optional 12 months prolongation).
