1. Ofertę sporządza się, pod rygorem nieważności, w postaci elektronicznej i opatruje się kwalifikowanym podpisem elektronicznym oraz w języku polskim.

3. Ofertę należy złożyć według wzoru formularza ofertowego oraz wymaganych załączników dołączonych do SIWZ, które winny być wypełnione i podpisane kwalifikowanym podpisem elektronicznym przez osobę (osoby) uprawnioną (uprawnione) do składania ofert i reprezentowania Wykonawcy, zgodnie z formą reprezentacji Wykonawcy określoną w rejestrze lub innym dokumencie, właściwym dla danej formy organizacyjnej Wykonawcy albo przez upełnomocnionego przedstawiciela Wykonawcy. Pełnomocnictwo winno być sporządzone w oryginale, w postaci elektronicznej i opatrzone kwalifikowanym podpisem elektronicznym.
