3. The Contracting Authority will consider that the condition relating to economic and/or financial standing has been met if the Contractor attaches to its tender a signed declaration on honour (Annex 2 to the EWZ) and:

3.1. proves by a document confirming that he is insured against civil liability in respect of the activities carried out in connection with the subject-matter of the contract — with a value of at least PLN 200 000.00 — relates to task No 1

III.1.3)Technical and professional ability

4. The contracting authority will consider that the condition relating to technical or professional capacity has been met if the Contractor attaches to its tender a signed declaration on honour (Annex 2 to the EWZ) and:

4.1 shows a minimum of 1 medical waste treatment services provided to hospitals, each with a value of at least PLN 200 000.00 a year — relates to task No 1

4.2 demonstrates that they have at least 2 persons holding ADR certificates authorising drivers to carry dangerous goods — relates to task No 1

4.3 demonstrates that it has at least 2 means of transport for dangerous goods — relates to task No 1

A- Statements/documents/subject matter/means of proof which the Contractor is required to submit together with the tender to the Tender Platform by the date and time set for the submission of tenders:

1. Tender form — Annex 1 to the ToR (form of a document — original in the form of an electronic document, signed by a qualified electronic signature of the contractor or authorised representative of the contractor);

2. Proof of payment of the security (form of the document — security lodged in money is to be paid by transfer to the Contracting Authority’s deposit account: BGK Kielce Branch, number: 15 1130 1192 0027 6001 1320 0011, with the words ‘A study in case No ZP-0586-2022-task No...’, and proof of payment in original form is recommended to be attached to the tender):

2.1. Note: if a guarantee is lodged in other forms accepted by the Contracting Authority (as set out in Part XI of the CTS), an original document in electronic form signed by the persons authorised to issue it by the guarantor/guarantor, i.e. the issuer of the guarantee/guarantee, must be attached to the tender via the Platform. An electronic copy (scan) of a document signed by a qualified electronic signature shall not be considered as the original guarantee.

3. An economic operator which relies on the capacities or the situation of other entities must prove to the Contracting Authority that, when performing the contract, it will have at its disposal the necessary resources of those entities, in particular by providing a written undertaking from those entities to place at its disposal the necessary resources for the performance of the contract as set out in Annex 7 to the ToR — Commitment.

4. In the event of joint competition by the Contractors, the tender shall be accompanied by a power of attorney from a person(s) to represent the Contractors in the procurement procedure or to represent the Contractors in the procedure and the conclusion of the public procurement contract, together with a declaration indicating which part of the contract each contractor will perform.

5. Any power of attorney must be submitted in the original form of an electronic document signed with the contractor’s qualified electronic signature or an electronic copy of the written original of the power of attorney authenticated by a notary with a qualified electronic signature.

B. The means of proof in question.

1. In order to prove that the supplies offered meet the requirements specified by the contracting authority, the contracting authority shall request the means of proof in question:

1.1. Safety Data Sheets, catalogues or other documents describing the range offered to enable verification of the conformity of the product with the requirements of the Contracting Authority — relate to Tasks Nos 2 and 3;

1.2. samples: in quantities:– 10 pieces of plastic bags (Task No 2)
