List and brief description of conditions:

Tiekėjai (taip pat visi tiekėjų grupės nariai, jei pasiūlymą pateikia tiekėjų grupė) ir ūkio subjektai, kurių pajėgumais remiasi tiekėjas, turi pateikti Europos bendrąjį viešųjų pirkimų dokumentą – aktualią deklaraciją, pakeičiančią kompetentingų institucijų išduodamus dokumentus ir preliminariai patvirtinančią, kad nėra tiekėjo pašalinimo pagrindų, nustatytų vadovaujantis Viešųjų pirkimų įstatymo 45 straipsnio nuostatomis. Europos bendrasis viešųjų pirkimų dokumentas parengiamas pagal standartinę formą, kuri yra patvirtinta 2016 m. sausio 5 d. Komisijos įgyvendinimo reglamentu (ES) 2016/7, kuriuo nustatoma Europos bendrojo viešųjų pirkimų dokumento standartinė forma (OL 2016 L 3, p. 16). Tiekėjai, pildydami Europos bendrąjį viešųjų pirkimų dokumentą elektroniniu būdu, naudojasi Europos Komisijos sukurta elektronine forma.

Tiekėjas per perkančiosios organizacijos nurodytą terminą turi pateikti pirkimo sutarties įvykdymo užtikrinimą patvirtinantį dokumentą. Pirkimo sutarties įvykdymas turi būti užtikrintas Lietuvos Respublikoje ar užsienyje registruoto banko ar kredito unijos garantija arba draudimo bendrovės laidavimo raštu. Pirkimo sutarties įvykdymo užtikrinimo vertė – 5 proc. nuo tiekėjo pasiūlyme nurodytos bendros pirkimo sutarties kainos EUR su PVM.
