III.1)Conditions for participation
III.1.1)Suitability to pursue the professional activity, including requirements relating to enrolment on professional or trade registers
List and brief description of conditions:

1. Ofertantii, tertii sustinatori si subcontractantii nu trebuie sa se regaseasca in situatiile prevazute la art.164, 165, 167 din Legea nr.98/2016; Modalitatea prin care poate fi demonstrata indeplinirea cerintei: se va completa DUAE de catre operatorii economici participanti(inclusiv eventualii terti si subcontractanti) la procedura de atribuire cu informatiile aferente situatiei lor. Documentele justificative care probeaza indeplinirea celor asumate prin completarea DUAE urmeaza a fi prezentate, la solicitarea AC, doar de catre primii trei ofertanti clasati in clasamentul intocmit la finalizarea etapei de licitatie electronica. Aceste documente pot fi:

— Certificate constatatoare privind lipsa datoriilor restante cu privire la plata impozitelor, taxelor sau a contributiilor la bugetul general consolidat(bugetul local, bugetul de stat) la momentul prezentarii,

— Cazierul judiciar al operatorului economic si al membrilor organului de administrare, de conducere sau de supraveghere al respectivului operator economic, sau a celor ce au putere de reprazentare, de decizie sau de control in cadrul acestuia, asa cum rezulta din certificatul contatator emis de ONRC/ actul constitutiv,

— Dupa caz, documente prin care se demonstreaza faptul ca operatorul economic poate beneficia de derogarile prevazute la art. 166 alin. (2),art. 167 alin. (2), art. 171 din Legea nr. 98/2016 privind achizitiile publice,

— Alte documente edificatoare, dupa caz.

2. Neincadrarea in situatiile prevazute de art.58 – 63 din Legea nr.98/2016 - Persoanele cu functie de decizie din cadrul autoritatii contractante in ceea ce priveste organizarea, derularea si finalizarea procedurii de achizitie sunt: Dr.Iman Radu Vasile – Manager, Dr.Pasarin Constantin – Director Medical, Ec.Tilea Simona – Director Financiar – Contabil, As. Se va completa DUAE de catre operatorii economici participanti la procedura de atribuire cu informatiile aferente situatiei lor. Operatorii economici ce depun oferta trebuie sa dovedeasca o forma de inregistare in conditiile legii din tara de rezidenta, din care sa reiasa ca op.economic este legal constituit, ca nu se afla in niciuna dintre situatiile de anulare a contituirii precum si faptul ca are capacitatea profesionala de a realiza activitatiile care fac obiectul contractului. Modalitatea prin care poate fi demonstrata indeplinirea cerintei: se va completa DUAE de catre operatorii economici participanti la procedura de atribuire cu informatiile aferente situatiei lor. Documentele justificative care probeaza indeplinirea celor asumate prin completarea DUAE, respectiv certificat constatator emis de ONRC, sau in cazul ofertantilor straini documente echivalente emise in tara de rezidenta, urmeaza a fi prezentate, la solicitarea AC, doar de catre ofertantul aflat pe primul loc in clasamentul intocmit la finalizarea etapei de licitatie electronica

Loturile: 1,2,3,4,5,6,7 Cifra de afaceri anuala generala Media cifrei de afaceri globale a ofertantului pentru anii 2017, 2018 si 2019 trebuie sa fie cel putin egala cu valoarea estimata a contractului(fara TVA) x 2. Pragul minim solicitat privind cifra de afaceri globala pe anii 2017, 2018 si 2019 pentru fiecare lot in parte este urmatorul: Lotul 1 = 110400.00 RON; Lotul 2 = 1408000.00 RON; Lotul 3 = 1240000.00 RON; Lotul 4 = 500000.00 RON; Lotul 5 = 112640.00 RON; Lotul 6 = 550000.00 RON; Lotul 7 = 64000.00 RON;

Minimum level(s) of standards possibly required:

Se va completa DUAE de catre operatorii economici participanti la procedura de atribuire cu informatiile aferente situatiei lor.

III.1.3)Technical and professional ability
List and brief description of selection criteria:

Loturile: 1,2,3,4,5,6,7 Pentru contractele de achizitie de bunuri: executarea de livrari de tipul specificat Prezentarea principalelor livrari de produse similare efectuate in ultimii 3 ani cu o valoare cel putin egala cu valoarea estimata a contractului. Ultimii 3 ani se raporteaza la data limita de depunere a ofertelor. Pragul minim solicitat privind produsele similare livrate in ultimii 3 ani pentru fiecare lot in parte este urmatorul: Lotul 1 = 55200.00 RON; Lotul 2 = 704000.00 RON; Lotul 3 = 620000.00 RON; Lotul 4 = 250000.00 RON; Lotul 5 = 56320.00 RON; Lotul 6 = 275000.00 RON; Lotul 7 = 32000.00 RON; Prin produse similare se întelege: dezinfectanti si/sau antiseptice, indiferent de forma de prezentare (solutii, geluri, spume, tablete) si fara a conditiona domeniul în care au fost livrate (sanitar, alimentar, zootehnic, purificare apa, colectivitati).

Documentele justificative care probeaza indeplinirea cerintei vor fi prezentate, la solicitarea autoritatii contractante, de ofertantii clasati pe primele 3 locuri in clasamentul intocmit la finalizarea etapei de licitatie electronica. Aceste documente justificative pot fi: liste de verificare/rapoarte interne cuprinzand punctele de control si criteriile de monitorizare a produselor si proceselor pe lantul de aprovizionare sau orice alte documente pe care ofertantul le considera relevante in indeplinirea cerintei.

Etapa finala de licitatie electronica se va desfasura prin intermediul SEAP si va fi initiata dupa o prima evaluare integrala a ofertelor. La etapa finala de licitatie electronica vor fi invitati sa participe doar operatorii economici ale caror oferte au fost declarate admisibile. Invitatia se transmite pe cale electronica, simultan, tuturor operatorilor invitati sa participe (prin e-mail generat si transmis automat de catre operatorul SEAP pe adresa de e-mail a ofertantului care are dreptul sa participe la etapa finala de licitatie electronica si totodata postata in cadrul sectiunii Notificari de sistem, sectiune accesibila din contul utilizatorului caruia i se adreseaza, dupa autentificare).

Numar de runde: 1. Durata rundei: 1 zi lucratoare (24 de ore). Ofertantii vor avea la dispozitie informatiile privind pozitia pe care o ocupa in clasament pe parcursul desfasurarii rundei de licitatie electronica. Ofertantii pot vedea cea mai buna oferta. Ofertantii pot vedea numarul de participanti inscrisi la licitatie. Pasul minim obligatoriu de licitare (in procent care se va aplica la valoarea estimate sau in suma fixa calculate prin aplicarea procentului la valoarea estimata) este 0,01 %. Dupa licitatia electronica ofertantul declarat castigator va avea ovligatia de a transmite o noua oferta financiara semnata si stampilata. La etapa finala a acestei faze, sistemul informatic va pune la dispozitia autoritatii contractante clasamentul final.
