1. Warunkiem udziału w postępowaniu o udzielenie zamówienia jest wniesienie wadium w prawidłowej wysokości dla danej części na którą Wykonawca składa ofertę. Kwota wadium dla ofert całkowitych wynosi 21 220,00 zł, a dla ofert częściowych zgodnie z poniższym wykazem:

Część 1: 3 000,00 zł

Część 2: 5 000,00 zł

Część 3: 850,00 zł

Część 4: 850,00 zł

Część 5: 920,00 zł

Część 6: 800,00 zł

Część 7: 250,00 zł

W przypadku złożenia ofert na kilka części, kwota wadium stanowi sumę wadiów ustalonych dla poszczególnych części zamówienia.

2. Wadium może być wniesione w:

1) pieniądzu,

2) gwarancjach bankowych,

3) gwarancjach ubezpieczeniowych

4) poręczeniach udzielanych przez podmioty, o których mowa w art. 6b ust. 5 pkt 2 ustawy z dnia 9 listopada 2000 roku o utworzeniu Polskiej Agencji Rozwoju Przedsiębiorczości (art. 45 ust. 6 ustawy).

3. Wadium wnosi się przed upływem terminu składania ofert.

4. Wadium wnoszone w pieniądzu wpłaca się przelewem na poniższy rachunek bankowy Zamawiającego: Bank Gospodarstwa Krajowego nr: 71 1130 1121 0006 5499 6720 0009. Wykonawca wnoszący wadium w pieniądzu zobowiązany jest do wpłacenia go odpowiednio wcześniej, tak aby znalazło się ono na koncie Zamawiającego przed datą i godziną składania ofert.

5. Wadium wnoszone w formie niepieniężnej powinno być wystawione na Uniwersyteckie Centrum Kliniczne, ul. Dębinki 7, 80-952 Gdańsk. W przypadku wniesienia wadium w formach określonych w punkcie 2 lit 2) – 2 lit 4) wykonawca przekazuje zamawiającemu oryginał gwarancji lub poręczenia, w postaci elektronicznej.

6. Zamawiający zatrzymuje wadium wraz z odsetkami, a w przypadku wadium wniesionego w formie gwarancji lub poręczenia, o których mowa w art. 97ust.7 pkt 2–4, występuje odpowiednio do gwaranta lub poręczyciela z żądaniem zapłaty wadium, jeżeli:

1) wykonawca w odpowiedzi na wezwanie, o którym mowa wart.107 ust. 2 lub art. 128 ust.1, z przyczyn leżących po jego stronie, nie złożył podmiotowych środków dowodowych lub przedmiotowych środków dowodowych potwierdzających okoliczności, o których mowa w art. 57 lub art. 106 ust.1, oświadczenia, o którym mowa w art.125 ust.1, innych dokumentów lub oświadczeń lub nie wyraził zgody na poprawienie omyłki, o której mowa w art. 223 ust.2 pkt 3, co spowodowało brak możliwości wybrania oferty złożonej przez wykonawcę jako najkorzystniejszej;

2) wykonawca, którego oferta została wybrana:a)odmówił podpisania umowy w sprawie zamówienia publicznego na warunkach określonych w ofercie,b)nie wniósł wymaganego zabezpieczenia należytego wykonania umowy;

3) zawarcie umowy w sprawie zamówienia publicznego stało się niemożliwe z przyczyn leżących po stronie wykonawcy, którego oferta została wybrana.

7. Zamawiający zwróci wadium dla Wykonawcy na zasadach określonych w art. 98 ustawy PZP.
