Mit der Angebotsabgabe sind folgende Nachweise und Erklärungen vorzulegen:

- Eigenerklärung des Unternehmens gemäß §§ 123, 124 GWB

- Referenzliste mit der Angabe der wesentlichen in den letzten drei Jahren erbrachten Leistungen

(Angabe des Umfangs der Leistung/Auftragswert, Ausführungsort, Ausführungszeit, Auftraggeber mit Adressdaten (auf Anforderung sind Ansprechpartner, Telefonnummer und E-Mail-Adresse sowie zur Konkretisierung des Auftrags die Auftrags-, Rechnungs- oder Kundennummer zu benennen))

- Bescheinigungen über die berufliche Befähigung der Leitung und Mitarbeiter anhand von Zeugnissen,

Nachweisen, Fortbildungsbescheinigungen oder anderen Bescheinigungen über berufliche Tätigkeiten mit

Angabe der Personen (besonders zu beachten: die Schnittmaßnahmen an Gehölzen sind nur mit geschultem

Personal durchzuführen)

- Angabe der sachkundigen Person/-en im Bereich Arbeitsschutz und eine schriftliche Erklärung, dass die

Mitarbeiter zum Thema Arbeitsschutz jährlich unterwiesen werden

- Aufstellung des vorhandenen Fuhrparks (Fahrzeuge, Maschinen, Kleingeräte usw.) mit Angabe der in der

Wertungsmatrix angegebenen Kriterien EuroNorm (inkl. Angabe der Kennzeichen) und Akkubetriebene Kleingeräte

1. Die Unterlagen stehen ausschließlich als kostenloser Download unter http://www.vergaben-wirtschaftsregion-aachen.de/VMPSatellite/company/welcome.do zur Verfügung. Die Weitergabe der Ausschreibungsunterlagen an Dritte sowie die gewerbliche Nutzung der Ausschreibungsunterlagen sind nicht gestattet.

2. Auskünfte über die Vergabeunterlagen sind schriftlich über das Vergabeportal der Wirtschaftsregion Aachen zu beantragen!

3. Die Vergabe der Lose ist beschränkt auf maximal zwei Lose pro Bieter.

Für den Fall, dass eine Firma nach der Wertung auf mehr als zwei Lose den Zuschlag bekommen würde, werden die zwei Lose mit der größten Auftragsgröße/ - summe an diese vergeben. Die weiteren Firmen erhalten den Zuschlag nach dem gleichem Prinzip bis alle 6 Lose vergeben sind.

Tritt der Fall ein, dass mehrere Firmen gleichbewertet sind, so entscheidet das Los, wer den Zuschlag erhält. Ggfs. wird dies für jedes zu vergebende Los durchgeführt.
