Avand in vedere ca s-au primit urmatoarele referate de necesitate:

Nr.686.X.116-01.07.2020, intocmit de Dr. Danciu Nicolae Danut in calitate de Sef Serviciu Medicina Legala;

Nr.76/25.06.2020 intocmit de Tic Garofita Rozalia in calitate de As. Sef Sectia Dializa;

Nr. 252/15.07.2020- intocmit de Dr. Moldovan Adriana in calitate de Medic Sef UPU-SMURD

Ca urmare,s-a intocmit un referat centralizat cuprinzand toate produsele aferente referatelor sus mentionate,inclusiv cantitati necesare pentru Spital.

Referat centralizat nr. 12262/15.06.2020, intocmit de Peres Nicoleta in calitate de Asistent Sef UPU-SMURD.

In cadrul procedurii vor fi achizitionati dezinfectanti si insecticide, necesari pentru dezinfectia sectiilor din cadrul Spitalului, UPU-SMURD SI Medicina legala, conform Anexei atasate strategiei de contractare cat si conform caietului de sarcini; Loturile 1,2,3,4,5 intra sub aplicabilitatea Ordinului 1082/2016. Pentru Lotul 6 NU se aplica ordinul 1082/2016.

Achizitionarea dezinfectantilor si insecticidelor este necesara pentru o buna functionare a Spitalului Judetean de Urgenta Satu Mare,care este unitate sanitara cu paturi, care participa la asigurarea starii de sanatate a populatiei,prin dezinsectie si dezinfectie.

Nu ne putem permite punerea in pericol a sanatatii si vietii pacientilor prin expunerea la riscuri asociate infectiilor.Astfel, in contextul celor sus mentionate,cu scopul asigurarii aprovizionarii cu dezinfectanti si insecticide, autoritatea contractanta organizeaza prezenta procedura de achizitie.

Pentru prodsele identificate de UPU - Spital: Aceste produse au facut obiectul Licitatiei de" DEZINFECTANTI SI ANTISEPTICE RELICITATE 2018" nefiind atribuite, motiv pentru care este necesara demararea unei proceduri care sa cuprinda aceste produse care asigura dezinfectia si dezinsectia.

Pentru produsele identificate de Sectia de dializa: Achizitionarea dezinfectantilor pentru aparatura de hemodializa necesare derularii Programului national de Dializa privind pacientii cu insuficienta renala cronica.Avand in vedere numarul mare de pacienti dializati atat cronici cat si acuti care este in crestere.In cadrul SJUSM se realizeaza aproximativ 400 de dialize /luna iar dupa fiecare sedinta de hemodializa se face dezinfectia timp de 1 ora.

Pentru produsele identificate de Serviciul Medico-Legal Satu Mare: Acest produs a facut obiectul Licitatiei de" DEZINFECTANTI SI ANTISEPTICE RELICITATE 2018" nefiind atribuit, motiv pentru care este necesara demararea unei proceduri care sa cuprinda acest produs care asigura dezinfectia chirurgicala a mainilor prin spalare cu ingrediente de protectie a pielii.

III.1)Conditions for participation
III.1.1)Suitability to pursue the professional activity, including requirements relating to enrolment on professional or trade registers
List and brief description of conditions:

Cerinta 1: Ofertantii/ asociatii/tertii sustinatori/subcontractantii nu trebuie sa se regaseasca în situatiile prevazute la art.164, 165, 167 din Legea nr.98/2016.

Modalitatea prin care poate fi demonstrata indeplinirea cerintei:

a) Ofertantii vor completa cu informatiile aferente situatiei lor fisierul DUAE, document care se completeaza direct in SICAP.

b) Documentele justificative care vor proba îndeplinirea celor asumate prin completarea DUAE vor fi prezentate la solicitarea autoritatii contractante, doar de catre ofertantii clasati pe primele 3 locuri, dupa aplicare criteriului de atribuire.

Aceste documente sunt:

— Certificat de atestare fiscala privind lipsa datoriilor restante referitoare platii impozitelor, taxelor sau a contributiilor la bugetul general consolidat (sau alte documente echivalente pentru operatorii economici straini), valabile la data depunerii – semnat cu semnatura electronica extinsa bazata pe un certificat calificat eliberat de un furnizor de servicii de certificare acreditat,

— Cazierul judiciar al operatorului economic și al membrilor organului de administrare, de conducere sau de supraveghere al respectivului operator economic, sau a celor ce au putere de reprezentare, de decizie sau de control in cadrul acestuia, asa cum rezulta din certificatul constatator emis de ONRC (sau alte documente echivalente pentru operatorii economici straini) – semnat cu semnatura electronica extinsa bazata pe un certificat calificat eliberat de un furnizor de servicii de certificare acreditat.

Modalitatea prin care poate fi demonstrata indeplinirea cerintei:

a) Ofertantii vor completa cu informatiile aferente situatiei lor fisierul DUAE, publicat de autoritatea contractanta in cadrul Documentatiei de atribuire, dupa care il vor depune in SEAP – semnat cu semnatura electronica extinsa bazata pe un certificat calificat eliberat de un furnizor de servicii de certificare acreditat.

b) Documentele justificative care vor proba îndeplinirea celor asumate prin completarea DUAE vor fi prezentate la solicitarea autoritatii contractante, doar de catre ofertantii clasati pe primele 3 locuri in clasamentul intermediar.

Aceste documente sunt:

— Declaratie conform Art 59-60 din Legea 98/2016.

Operatorul economic ce depune ofertă trebuie sa dovedeasca o forma de inregistrare in conditiile legii din tara de rezidenta, este legal constituit si nu se afla in niciuna dintre situatiile de anulare a constituirii, precum si faptul ca are capacitatea profesionala de a realiza activitătile care fac obiectul contractului.

Modalitatea prin care poate fi demonstrata indeplinirea cerintei:

a) Ofertantii vor completa cu informatiile aferente situatiei lor fisierul DUAE, document care se completeaza direct in SICAP.

Modalitatea de departajare a ofertantilor clasati pe acelasi loc, cu preturi egale:

În cazul în care criteriul utilizat este "preţul cel mai scăzut"

În cazul în care două sau mai multe oferte sunt situate pe primul loc, autoritatea contractantă solicită ofertanţilor o nouă propunere financiară, iar contractul va fi atribuit ofertantului a cărui nouă propunere financiară are preţul cel mai scăzut.La solicitarea Autoritatii contractante,operatorii economici vor transmite raspunsul lor online in SICAP. Iar pentru ofertantii clasati pe acelasi loc (cu referire la locurile 2-3) se va solicita reofertare a pretului online in SICAP, putand fi efectuata pana la limita ofertei clasate pe locul anterior, operatorii economici transmitand raspunsul lor in rubrica REOFERTARE din SICAP.

Termenul limita in care AC va raspunde in mod clar si complet tuturor solicitarilor de clarificari /informatii suplimentare va fi a 11 a zi inainte de data limita de depunere a ofertelor conform art.160 coraborat cu art.161 din Legea 98/2016 cu modificarile si completarile ulterioare.

Numar zile pana la care se pot solicita clarificari inainte de data limita de depunere a ofertelor/candidaturilor: 20 zile.
