Vision and Aims of the Service
We want Cornwall and the Isles of Scilly to be one of the best places for children and young people to thrive. The document that accompanies this Prior Information Notice offers the opportunity to build innovative community health services built around the Living Well principles, with a focus on prevention, early intervention and recovery and which put children, young people and their families at the centre of service delivery.
The service will play a vital part in ensuring every child has the best start in life, enabling all children and young people to maximise their potential to achieve their optimum health and well-being and secure a successful and healthy transition to adulthood.
The overall aims of a Children's Community Health and Well-being Service are to:
— Achieve excellent health and well-being outcomes for children and young people that are amongst the best in Europe.
— Deliver high quality seamless pathways of care that identify, assess, treat and support children and young people, their carer's and families when and where they need it.
— Ensure that where pathways involve different providers and services, that the linkages are robust, care delivery remains of a high quality and seamless to the child and their family.
— Deliver accessible and safe services on an equitable and consistent basis across Cornwall and the Isles of Scilly according to identified need.
— Provide efficient, effective, flexible and responsive operational models of service that align across health, social care and voluntary sector resources.
— Deliver a service that focuses on early help and prevention, and provides innovative approaches that reduce escalation and support recovery.
— Deliver long term opportunities and outcomes for our children and young people, (in line with the NHS 5 Year Forward Plan and Cornwall and Isles of Scilly Living Well approach).

The purpose of this document is to provide an overview of the services which the provider will be required to deliver. It provides a high level summary of the population needs, and a set of outcomes and objectives for the future provision.
The document describes the components of a complex matrix of services to promote health and well-being and some aspects of educational assessment which support children and young people and their families in Cornwall and the Isles of Scilly (CIOS).
Please note: This Prior Information Notice relates to NHS Kernow commissioned services. Potential bidders should note that a separate Prior Information Notice has been released by Cornwall Council in conjunction with The Council for the Isles of Scilly and NHS England for children's services commissioned by them. Cornwall Council, Council of the Isles of Scilly (CIOS), NHS England (NHSE) and NHS Kernow are working in close alignment to procure Children's Health and Well-being services in Cornwall and the Isles of Scilly. Although there are separate procurement processes, Cornwall Council, CIOS, NHSE and NHS Kernow will be looking for close alignment between the contracts.
To ensure each process relates to the other and that services from April 2016 have synergy, a joint Information event will be held on 16.12.2014 at Kingsley Village, Penhale, Fraddon, Cornwall, TR9 6NA starting at 10:00. Commissioners from each commissioning organisation will be available to discuss the services at this event.
