Accountable Care System (ACS) and Sustainability and Transformation Partnerships (STP) Innovation Partner Framework.
The NHS Five Year Forward View called for greater integration of healthcare services, of health and social care, and of physical and mental health services. Sustainability and Transformation Partnerships (STPs) and Accountable Care Systems (ACS) have been established to bring together NHS and Local Government partners to build services around the needs of whole populations and provide the best possible quality of care and health outcomes within a defined set of resources.
NHS England are looking to engage organisations who can work with STPs and ACS to accelerate this goal and to strengthen capabilities in Population Health Management (PHM) and digital transformation.

This PIN is not yet a call for competition. NHS England is issuing this PIN to support further engagement with interested suppliers of the below listed capabilities to explore the design of a ‘one stop shop’ framework and contracting vehicle to allow STP and ACS partners to more easily source a range of transformation support, including:
— Digitising care and operational services (organisational and place-based), including Electronic Patient Records — providing access for GDE fast followers;
— Health Information Exchange and Interoperability Hubs (governance support, technical software and infrastructure);
— IT support and transformation (incl. GP and Primary Care IT);
— Data Management and Information Governance support.
— Population Health Analytics which includes informatics and analytics to support:
o System planning, health needs assessment, assurance and evaluation;
o Care coordination, risk stratification and impactability modelling;
o Care management, clinical decision support and workflow.
Impact and Intervention support:
— Transformation and change which includes:
o Pathway enablement and care model design;
o Workforce development / OD / Leadership;
o System design and reform (including financial and contract reform)
— Primary Care transformation support for GP Forward View (Improving access, managing demand, telehealth, etc.)
— Patient empowerment and activation which includes:
o Self-care programmes, including scaling up Test Bed innovations and other transformation programmes involving innovative use of technology to share information, monitor and influence care interventions;
o Integrated Personal Commissioning and Personal Health Budgets
— System Optimisation support including:
o Demand management and capacity planning solutions, including Command Centres (intra and inter organisational);
o Provider networking (supply chain support), financial and quality management and continuous improvement;
o Medicines Optimisation and value programmes
This scope is subject to further engagement and may evolve. The framework is envisaged to be refreshed regularly to accommodate the evolving market in the UK and any changing requirements from the demand side.
Any potential Framework Agreement may be split into Lots, to be determined. Any future procurement activity will be the subject of a separate Contract Notice which will supersede this Notice and which will form the call for competition.

Any interested suppliers will be required to register via our online Bravo portal at https://nhsengland.bravosolution.co.uk/web/login.html. This portal will be the channel used to issue further information to interested suppliers.
To support the development of the scope of the Framework and to ensure a good understanding of interested suppliers and their offerings, suppliers will be requested to complete a short survey following registration. Further information and instructions regarding this survey will be provided to suppliers via the online portal.
