Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:
1. enkel voor perceel 1: verklaring betreffende de gemiddelde totale jaaromzet van de levering van de artikelen waarop perceel 2 betrekking heeft over de laatste drie boekjaren;
2. enkel voor perceel 2: verklaring betreffende de gemiddelde totale jaaromzet van de levering van de artikelen waarop perceel 2 betrekking heeft over de laatste drie boekjaren.
Minimum level(s) of standards possibly required:
1. De gemiddelde totale jaaromzet van de levering van de artikelen waarop perceel 1 betrekking heeft over de laatste drie boekjaren bedraagt minimaal 4 00 000 EUR incl. btw;
2. De gemiddelde totale jaaromzet van de levering van de artikelen waarop perceel 2 betrekking heeft over de laatste drie boekjaren bedraagt minimaal 235 000 EUR incl. btw.
III.1.3)Technical and professional abilityList and brief description of selection criteria:
Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:
1. per perceel een lijst van de voornaamste gelijkaardige leveringen waarop de opdracht betrekking heeft die hij gedurende de afgelopen drie jaar heeft verricht (te tellen vanaf de publicatiedatum van de opdracht), hun bedrag/hoeveelheden, data en de instanties waarvoor ze bestemd waren. Per instantie wordt vermeld: het onderwerp van de leveringen + de jaarlijkse omzet (excl. btw) per jaartal + de publiek- of privaatrechtelijke instanties (met vermelding van contactgegevens (naam/telefoonnummer/mailadres) waarvoor zij bestemd waren;
2. in het bezit zijn van een geldig certificaat ISO 13485 of gelijkwaardig;
3. een erkenning van de fabrikant als officiële verdeler;
4. een erkenning van de fabrikant om de herstellingen en onderhouden in eigen atelier door eigen personeel uit te voeren.
Minimum level(s) of standards possibly required:
1. In de lijst staan minimaal vier referenties per perceel, waarvan minstens één in een universitair ziekenhuis. De aanbestedende overheid behoudt zich het recht de opgegeven referenties te controleren;
2. Minimum één attest voorleggen.
3. Een geldig attest van erkenning voor het toestel dat zij aanbieden;
4. Een geldig attest van erkenning.
