Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

1. Referentielijst waarbij de inschrijver de afgelopen drie jaar (2019, 2020 en 2021) diensten verleende. De inschrijver vermeldt per referentie de omzet gedurende de afgelopen 3 jaar en de contactpersoon van de inschrijver.

2. De inschrijver en zijn onderaannemers dienen te beschikken over de nodige vergunningen ( exploitatie- en milieu- of omgevingsvergunning) en erkenningen ( door OVAM) voor het ophalen, transporteren en verwerken/verwijderen van de verschillende afvalsoorten opgenomen in dit bestek. Daartoe dient hij de nodige bewijzen van erkenning en vergunning bij de offerte te voegen met de vermelding van de geldigheidstermijn hiervan.

3. Alle door de aannemer opgehaalde afval dient opgehaald te worden door daarvoor erkende en opgeleide transporteurs. De respectievelijke firma’s en hun erkenningsnummers dienen in de offerte meegegeven te worden.

4. De verwerking en vernietiging dient, door de zorgen van de inschrijver, te gebeuren door een erkend vernietiger voor de opgehaalde afvalsoort.

1. Referentielijst: minimum 5 referenties, waarvan minimum 1 met een jaaromzet van EUR110.000 inclusief btw gedurende de afgelopen 3 jaar (2019, 2020 en 2021).

De inschrijver wordt verwacht een plaatsbezoek uit te voeren aan de door de aanbestedende overheden te kiezen locaties. Op straffe van nietigheid van zijn offerte, moet de inschrijver een plaatsbezoek bij ieder deelnemend bestuur uitvoeren. De inschrijver dient het bewijs/attest van het plaatsbezoek bij ieder deelnemend bestuur in bijlage ingevuld toe te voegen aan zijn offerte.

De inschrijver ziet de locaties na op het vlak van toegankelijkheid en bereikbaarheid van de ophaalplaats. Hierbij toetst de inschrijver de afmetingen van de ingang en aanrijwegen af t.o.v. de dimensie en draaicirkel van de afvalwagens. De inschrijver neemt de nodige voorzorgen en draagt de verantwoordelijkheid om omhalingen te garanderen.

De inschrijver dient het attest in bijlage correct ingevuld toe te voegen aan zijn offerte.
