Het UEA, waarmee de ondernemer verklaart dat hij zich niet bevindt in een van de uitsluitingssituaties als bedoeld in de artikelen 67 tot 69 van de wet van 17.6.2016.

III.1.2)Economic and financial standing
List and brief description of selection criteria:

Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

1. een verklaring betreffende de totale omzet en de omzet die verband houdt met de opdracht, over de laatste drie neergelegde boekjaren of afhankelijk van de datum waarop de kandidaat met zijn activiteit is begonnen en voor zover de betrokken omzetcijfers beschikbaar zijn op niveau van de Belgische vennootschap. Indien de kandidaat om redenen die hij moet verantwoorden, niet in staat is de gevraagde referenties voor te leggen, kan hij zijn economische en financiële draagkracht aantonen met andere documenten die de aanbestedende overheid geschikt acht.

Minimum level(s) of standards possibly required:

1. Deze omzet dient minstens gelijk te zijn aan twee keer de offerteprijs van de percelen waarvoor de inschrijver een offerte indient.

III.1.3)Technical and professional ability
List and brief description of selection criteria:

Het UEA, waarmee de ondernemer verklaart dat hij voldoet aan de onderstaande selectiecriteria:

1. lijst van klanten gedurende de afgelopen drie jaar met het totaal bedrag, excl. btw van de leveringen per jaar omzet in verhouding tot dit dossier;

2. lijst van klanten gedurende de afgelopen drie jaar met het totaal bedrag, excl. btw van de leveringen per jaar omzet in verhouding tot dit dossier;

3. enkel voor perceel 2: De technische- en veiligheidsfiche per aangeboden product;

4. enkel voor perceel 3: De technische- en veiligheidsfiche per aangeboden product;

5. enkel voor perceel 3: Een desinfectiemiddel (PT2-biociden, product type 2);

6. de inschrijver voegt bij zijn offerte de nodige vergunningen en certificaten die hem toelaten zijn producten in België op de markt te brengen;

7. de inschrijver moet aantonen dat alle aangeboden referenties voldoen aan de bepalingen uit de MDR 2017/745;

8. de inschrijver moet aantonen dat alle aangeboden referenties voldoen aan de bepalingen uit de MDR 2017/745.

Minimum level(s) of standards possibly required:

1. drie voornaamste klantreferenties;

2. drie voornaamste klantreferenties met een gelijkwaarde omvang als dit dossier;

3. De technische- en veiligheidsfiche per aangeboden product;

4. zie minimum informatie op veiligheidsfiche en technische fiche beschreven onder III. Technische bepalingen van perceel 3;

5. Een desinfectiemiddel (PT2-biociden, product type 2) moet goedgekeurd / geregistreerd zijn via de Belgische of Europese procedure;

6. Bij vergunningen en certificaten:

— zijn productie en/of distributievergunningen van het FAGG voor productie en/of distributie van medische hulpmiddelen in BELGIË,

— de registratie van zijn aangeboden producten bij het FAGG voor wat betreft alle producten toepasbaar onder de geneesmiddelenwetgeving,

— ISO 13485 certificering medische hulpmiddelen voor de toepasbare producten in alle percelen,

— de inschrijver moet aantonen via certificaten opgesteld door een notified body dat alle aangeboden referenties beschikken over een CE-markering en voldoen aan de Medical Device Directive,

7. Aanleveren van een MDR readyness-statement;

8. Aanleveren van een MDR readyness-statement.
